A 21-year-old female patient received BNT162b2 (COMIRNATY), in Oct2021 as dose 2, single (Batch/Lot number: unknown) for covid-19 immunisation; davesomeran, elasomeran (MODERNA COVID-19 VACCINE, BIVALENT (ORIGINAL AND OMICRON BA.

5)), on 18Feb2022 as dose 3 (booster), single (Batch/Lot number: unknown) for covid-19 immunisation.
The patient's relevant medical history included: "antiphospholipid syndrome" (unspecified if ongoing), notes: as a 15-year-old; "appendix removed" (unspecified if ongoing), notes: as a 15-year-old.
The patient's concomitant medications were not reported.
Vaccination history included: Comirnaty (dose 1, single), administration date: Sep2021, for COVID-19 immunisation.

The patient underwent the following laboratory tests and procedures: Scan: Unknown results.
Therapeutic measures were taken as a result of cardiac failure, myocardial infarction, myocarditis, malaise, fatigue, illness, abdominal pain upper, vomiting, pyrexia, altered state of consciousness, dyspnoea.
The patient date of death was 27Mar2022.
Reported cause of death: "died of heart failure", "myocardial infarction", "Myocarditis/subacute myocarditis".
Clinical course: It was reported that patient received two doses of Pfizer vaccine in Sep2021 and Oct2021, followed by the Moderna booster on 18Feb2022, in order to keep her job.
The morning after her booster, patient fainted in her bedroom, falling and hitting her head on her ensuite cabinet.
Patient's mother rang the Covid vaccine helpline and stated that "they were dismissive, telling me to call an ambulance if I think I need one" hence patient's mother monitored her.
On an unspecified date patient felt sick and tired and continued to be sick for the next six days with stomach pain, vomiting and a fever.
On 05Mar2022, patient was experiencing difficulty breathing and drifting in and out of consciousness so an ambulance was called and patient was brought to the hospital where she was immediately diagnosed with heart failure and advised to be transferred to another hospital where she was admitted.
For the next three weeks, patient had numerous procedures and scans.
The patient died of heart failure on 27Mar2022, her death certificate also states she died of myocardial infarction and that she had subacute myocarditis.
Causality was reported as that the panel agreed with the TGA's assessment that the myocarditis the woman experienced was likely to have been related to vaccination given the available information, including the absence of other apparent causes of the myocarditis, and the time frame for the onset of symptoms.
However, the panel acknowledged there were several other complicating factors that may have contributed to her death The information on the batch/lot number for BNT162b2 has been requested and will be submitted if and when received.
