Narrative: Patient was not previously COVID-19 positive and did not have any predisposing factors(PMH, allergies, etc.
) for experiencing an adverse drug event.
The ADR did not occur at the time of the administration of the vaccine nor was there an ADR that occurred between the observation period and the date of death.
Patient was 87 and patient was in rehab in Feb 2021 in slow multi organ failure requesting d/c to hospice.
Patient was in hospice 2/13-2/19 for severe heart failure resulting in 40 pound fluid weight gain since November 2020 before returning to ER on 2/19/21 after wishes to restart aggressive life sustaining treatment vs.
palliative approach.
He was stabilized and discharged home on 3/3/21.
Patient was readmitted on 3/16/21 after his condition continued declining at home.
His heart and kidney failure progressed while inpatient until he passed away on 3/20.

His Comorbidities include chronic afib on apixaban, severe mitral regurgitation, aortic stenosis, AAA, OSA, CKD, HTN, CHF.
