증상 상세
The patient came to our offsite COVID-19 vaccination clinic.
She denied previous vaccination and requested Novavax.
Initial Impact search by receptionist showed no previous history of vaccination for COVID-19.
Patient was screened by three individuals who asked if she previously received a COVID-19 vaccination; she denied each time.
After the clinic, when her vaccination was being entered into Impactsiis,, it was discovered that the patient had previously received a primary series of Moderna (3/23/21 and 4/20/2021) and a booster of Pfizer (11/30/2021).
Patient did not report any side effects post vaccination during the observation period.
Several attempts were made to contact the patient (phone call, text, email) in order to clarify her record; unable to reach patient.
